Stem cells are non-differentiated cells. They can develop into various “specialist” cells in the body. Doctors are already using stem cells in some treatments. With further research, experts expect stem cell transplants to have more uses and new treatments. In this article, we’ll explain what a hematopoietic stem cell transplant (HSCT) is and how it is used to treat certain types of cancer and other diseases.
Hematopoietic cell transplant is a medical procedure used to treat a variety of diseases, including childhood cancer. It replaces the damaged hematopoietic cells with healthy blood-forming cells.
Since the bone marrow is the main source of blood-forming cells, this procedure is traditionally called a bone marrow transplant. With gradual development in this field it is now known as hematopoietic cell transplant. These words are often used interchangeably. Some people also call this procedure a stem cell transplant.
Childhood cancer can be successfully treated with a transplant, but it can have serious side effects and delayed effects. This option should be carefully considered. Getting a transplant is not an easy process. For this it is very important to have an experienced doctor.
Who donates stem cells?
For this, stem cells can be taken from the donor or the patient’s own body. In allogeneic HSCT, a donor supplies the stem cells. Their tissue and blood type must match the patient’s stem cells, so the donor is often a close relative or relative.
Doctors examine a protein called human leukocyte antigen to compare blood and tissue types and check whether the stem cells of the two match. In autologous HSCT, stem cells are passed into the body in a targeted manner to their cells.
What is the role of bone marrow and blood-forming cells?
The bone marrow is a soft, sponge-like tissue at the center of most bones in the body. A large number of blood-forming (hematopoietic) cells reside in the bone marrow. Hematopoietic cells are the parent of all other blood cells. When mature these cells are formed and later become the following:
- red blood cells (which carry oxygen)
- Platelets (which help in clotting of blood)
- white blood cells (which fight infection)
The bone marrow functions like a blood cell factory, constantly producing new hematopoietic cells so that red blood cells, white blood cells, and platelets can do their job in the blood. The bone marrow acts as a blood cell reservoir, constantly producing new hematopoietic cells so that red blood cells, white blood cells, and platelets can do their job in the blood.
All red blood cells and platelets, as well as about 70 percent of white blood cells, are made in the bone marrow.
A small number of hematopoietic cells can be found in peripheral (circulating) blood. Cord blood is also a source of blood-forming cells.
The goal of the transplant is to:
Give high-level chemotherapy and/or radiation to destroy the patient’s cancer
Prevent rejection of donor cells (above rejection)
Replace the patient’s blood cells with healthy donor cells that will fight and destroy the cancer cells.

How is cancer transplanted during childhood?
Transplants are mainly used to treat leukemia (cancer of the blood) if someone has had cancer in childhood, usually when the patient’s cancer treatment has failed. Leukemia, a cancer of the blood and bone marrow, causes the body to produce damaged white blood cells, making the patient very ill.
Transplants are sometimes used to treat other cancers as well. eg neuroblastoma, multiple myeloma, recurrent Ewing sarcoma (recurrent ewing sarcoma) and recurrent Wilms tumor (recurrent Wilms tumor),
Transplants can also be used to treat patients with cancers that involve soft tissue or brain tumors that require chemotherapy or very high doses of radiation to treat the disease.
